This certifies that____________________________, who lives at ___________________ __________________________________________, _________________(city),_____________(state), ______________(zip), (___)____________(home phone) (___)_________________(work phone), is a member of the Mid-America Hunting Association, an Association of the State of Missouri and is entitled to all the rights and privileges as such, conferred by, and subject to, the Rules and Regulations of the Association. An irrevocable assent to the Rules and Regulations and any amendment of the Rules and Regulations is given by the holder of this membership by acceptance of this membership.

This membership can be transferred only in the manner prescribed by the Rules and Regulations, and its transfer is not final until shown on the books of the Association and an officer of the Association attested to the transfer with his signature. The Association reserves the discretion to deny any transfer of membership. Membership cannot be advertised for sale or transfer in any form of public media.

In consideration for the this agreement, the member shall be entitled to use of Association facilities provided by the Association as set forth in the Member's Manual, a copy of which has been delivered to member herewith and receipt hereby acknowledged by member. Member's rights are subject to payment of initial enrollment fees, annual membership dues and any other expenses or guest fees due to the Association. Failure to pay any fees or comply with all Rules and Regulations as agreed herein, may result, at the discretion of and best interests of the Association, in forfeiture all amounts paid and all membership rights.

Acceptance of membership into the Association is subject to an annual membership fee which can be increased on an annual basis at the reasonable discretion of the Association. Participation by a member in any of the Association facilities in years subsequent this initial term shall be deemed consideration for renewal of this agreement. Membership renewal dues for the first year subsequent initial dues will commence on___/___/___ in the amount of $_____________ plus sales tax.

Member accepts this agreement for the sole and express purpose of availing himself of the Association facilities, and by affixing his signature to this Agreement, states that he has read, understands and consents that this Agreement and the Membership Manual which contains the Association's Rules and Regulations therein, represent the entire agreement between the parties.
